What is Nursing Home Transition?
Nursing Home Transition is the voluntary transfer of an eligible Medicaid beneficiary residing in a nursing home to a community setting such as a family member’s home, an apartment or home, an Assisted Living Facility or Adult Family Care Home.
Who is eligible for Nursing Home Transition services?
You may be eligible for transition services if:
- You are age 18 or older
- You are Medicaid eligible
- You are able to live safely in the community
What services will I receive if I transition?
There are different programs for those who transition. Each program offers different services. Examples of services you may receive include:
- Case management
- Assistance with activities such as help with bathing, dressing and eating
- Referrals to agencies that provide home-based services such as Aging Resource Centers and Centers for Independent Living
- Help getting community services like Meals on Wheels
- Help finding housing
In addition, you will retain all of the medical services you are eligible for under regular Medicaid.

What happens next?
A trained specialist will talk with you about Nursing Home Transition. The specialist will collect basic information from you and will ask if you have friends or family members who will be able to help with some of your care. The specialist will provide the collected information to the appropriate agency for follow-up.
Next, a case manager will meet with you, members of your family, and any other people you choose. The case manager will explain your options and discuss your individual needs. Before you can move out of the nursing home, your case manager will need to be sure that you can safely live in your home or community.
How long will the Nursing Home Transition process take?
Your case manager will need to develop a plan of care and set up the services you need. This may take several weeks or longer in some cases. You and your family should be part of this process to ensure a safe and successful transition.
Will Medicaid pay my rent if I move into an apartment?
Medicaid cannot pay your rent. Also, Medicaid will not pay for security deposits, utility deposits or telephone deposits.
If I move into an Assisted Living Facility or Adult Family Care Home, will Medicaid pay my entire bill?
Medicaid can pay for the services you receive in an Adult Family Care Home or Assisted Living Facility. Medicaid cannot pay for the room and board portion of the charges in an Assisted Living Facility or Adult Family Care Home.
What happens if I change my mind or I am not able to stay in the community?
You can change your mind at any time during the transition process. If you have moved to the community and want to return to the nursing home, your case manager will help you locate a bed in the facility you left or in another facility of your choice.
What else do I need to know?
Each individual’s situation is reviewed on a case by case basis. The services you may need to stay in the community are provided by community service agencies, Medicaid waiver programs, and help from your family members and friends.
