Online License Renewal Application The Agency for Health Care Administration (AHCA) is introducing a new method for license renewals: online applications. This system will let providers renew a license on the computer.
If you are a Medicaid provider, you may have a separate obligation to notify the Medicaid program of a name/address change, change of ownership or other change of information. Please refer to your Medicaid handbooks for additional information about Medicaid program policy regarding changes to provider enrollment information.
Renewal Reminder Notices are no longer being sent by certified letter. You will now receive a renewal reminder postcard at least 90 days prior to the expiration of your license. To ensure that you receive timely notice, please visit www.FloridaHealthFinder.gov to verify that your mailing address is accurate. Please feel free to contact your licensing unit representative if you have any questions.
NOTE: Agency letters are often sent to applicants by certified mail. All letters are sent to the mailing address provided on the application.

Health Care Advance Directives
Minimum Data Set (MDS): A standardized definition of items used to improve quality of care by assessing the needs and functional status of residents in long term care facilities that participate in the Medicare and/or Medicaid program.
Outcome Assessment Information Set (OASIS): A uniform core set of data elements used to improve quality of care by enabling measurement of patient outcomes in home health agencies that participate in the Medicare and/or Medicaid program.
