HOME HEALTH AGENCY QUARTERLY REPORT


All home health agencies must submit a quarterly report at the website on the items required in state law (400.474(6) (f), Florida Statutes).There are no exceptions.

INSTRUCTIONS AND FREQUENTLY ASKED QUESTIONS

Patient census as of the last day of the quarter:

  1. Number of insulin-dependent diabetic patients receiving insulin injection services
  2. Number of patients receiving home health services from my home health agency AND a licensed hospice provider
  3. Total number of patients receiving home health services
  4. The names and license numbers of nurses (RN’s or LPN’s) whose primary job responsibility is to provide home health services to patients and received remuneration in excess of $25,000 for the three months of the quarter. 

 

How do I submit the report?

  1. Get your user ID (HHA with your license number) and the password you created when you submitted the last report. If you do not have a password, please email HQAHomeHealth@ahca.myflorida.com to request a password giving your name, agency name and license number.

How can I ensure that AHCA has received my quarterly report?

When you submit your report on the web pages, you will receive immediate verification to your email address that your report was received. If you don’t receive an e-mail confirmation you need to see if the e-mail address in your “User Profile” in the web portal is correct. If not, you need to change your e-mail address. Click on “View User Profile” and correct your e-mail address.

 

Do I have to enter my data at the website?

Does this report include patients served by contract employees?
Yes

 

Does this report include patients served by home health agencies that we contract with to provide some of the services for our patients?
Yes

 

Does my home health agency have to complete the report if we do not provide any skilled services?
Yes

 

Does my home health agency have to complete the report if we are not a Medicare or Medicaid home health agency?
Yes

 

We contract with another home health agency, nurse registry or health care services pool for nurses. We do not know what nurses are paid each quarter. How can we complete the report?

The state law requires that your home health agency report all nurses who were paid for serving patients for your home health agency. Therefore, you will need to get information from the home health agency, nurse registry or health care services pool, such as an itemized invoice that gives the name and license number of the nurse, the service provided, the date(s) of the services and the amount paid.

 

Our Director of Nursing, the registered nurse designee in her absence, and other RN supervisors also provide some nursing services to patients. Do they have to be included in the report?

No. The law says home health agencies are to include nurses “whose PRIMARY job responsibility is to provide home health services to patients.”
