NURSE REGISTRIES
Update July 14, 2010: The new 59A-35 rule that regulates all entities licensed by AHCA went into effect. Please note that any changes you want made to your license including
- Address (at least 21 days advance notification prior to the move is required or a $500 fine will be assessed);
- adding or deleting counties;
- changing the name of your agency; or
- lost your license copy
will now require the nurse registry to send in selected sections of the Application AHCA Form 3110-7004 and a $25 fee. Refer to the Application Checklist Section D (AHCA Form 3110-7004 July 2009 or recommended August 2010).
